- Navigate to Account Menu (top right corner next to your Hub ID) > Account & Billing.
- Select Company & Contacts in the left sidebar menu.
- Click Edit name.
- In the dialog box, click Download Name Change Request Form.
- Fill out the request form. Instructions for completing the form are included in the PDF.
- When you've completed the form, click the Upload Name Change Request Form button in the dialog box.
- Click Submit Request.
- Your request will be reviewed. The countersigned form will appear in your Documents Center (Account Menu > Account & Billing > Document Center).
